Change personal details

Go to your online account and into personal details. You can then edit the details that need changing. If you need to update your registered home address, (this is the address registered with your GP surgery) please make contact with us here. 

You can amend your:

  1. First Name
  2. Surname 
  3. Title
  4. Gender

Please note- these details must match exactly the records your GP surgery hold.

Want to check the details your GP holds for you, you can do this on the NHS App. 

  • Log in 
  • Go to your homepage 
  • view your GP heath record
