I want to close my account

Due to a duty of care we are unable to close the account until we have verified the patient, patient’s representative or carer and completed the relevant identity checks.

To close your account and ensure your transition to your new pharmacy runs as smoothly as possible, we do need to speak with you.

Please call us on 0113 265 0222 and select option 4 for our cancellation line, you should then be placed immediately in the queue awaiting an agent to answer your call.

Please be aware that until you have made contact with ourselves regarding this, we may still receive prescriptions from the surgery.
